Clomiphene (Clomid): A Tracking Guide for the Fertility SERM

Clomiphene (Clomid) is a SERM long used to induce ovulation in women and, off-label, to raise testosterone and support fertility in men by stimulating the pituitary to release more LH and FSH. It is a common component of post-cycle therapy and of clinical protocols for secondary hypogonadism.

This is an educational tracking guide. Clomiphene is prescription-only. Its distinctive warnings - visual disturbances and, in fertility settings, ovarian hyperstimulation - shape what you monitor.

What it does

  • Blocks estrogen receptors at the hypothalamus and pituitary, which the body reads as low estrogen
  • In response, LH and FSH rise, stimulating the gonads
  • In men, this raises endogenous testosterone and supports sperm production
  • In women, it triggers ovulation

What to track daily

  • Dose, timing
  • Mood - clomiphene mood effects are commonly reported

  • Visual disturbances: blurring, floaters, light sensitivity, trails - stop and seek care if these appear

  • For fertility patients, any abdominal swelling or pain (OHSS warning in women)

  • Side-effect accumulation, especially visual symptoms

Bloodwork worth doing

  • Baseline: LH, FSH, total and free testosterone, sensitive estradiol (men); cycle-appropriate hormones (women)
  • Follow-up at 4-6 weeks: confirm LH, FSH, and testosterone are responding
  • Estradiol can rise as testosterone rises; trend it

Realistic expectations

  • Testosterone and LH and FSH typically rise within weeks when it works
  • Mood side effects are a real and common reason people stop
  • Visual disturbances, though uncommon, are a recognised reason to discontinue

Common mistakes

  • Ignoring or pushing through visual symptoms
  • Dismissing mood changes
  • No follow-up bloods to confirm it is actually working
  • Assuming it works equally well for everyone (response varies)

  • On: daily mood, libido, and crucially visual-symptom logs
  • Follow-up: recheck hormone panel at 4-6 weeks

Safety notes
