Anastrozole: A Tracking Guide for the Aromatase Inhibitor

Anastrozole is a non-steroidal aromatase inhibitor (AI): it blocks the enzyme that converts androgens to estrogen, lowering serum estradiol. It is a breast-cancer drug and is used off-label by men on testosterone or steroids to control estrogen-related side effects such as water retention and gynecomastia.

This is an educational tracking guide. Anastrozole is prescription-only. The defining lesson with any AI is that estrogen is not the enemy - over-suppressing it causes its own serious problems.

What it does

  • Inhibits aromatase, lowering estradiol
  • Reduces estrogenic side effects (bloating, gynecomastia risk) in those aromatising heavily on testosterone
  • Is dose-sensitive: small differences swing estradiol a lot

Why over-suppression is the real risk

Crashing estradiol too low causes:

  • Killed libido and erectile dysfunction
  • Achy, dry joints
  • Low mood and fatigue
  • Worsened lipids
  • Accelerated bone density loss over time

Many side effects people blame on "high estrogen" are actually low estrogen from over-dosing an AI.

  • Water retention and nipple symptoms (the things you may be treating)

  • Well-being and libido trend

Bloodwork worth doing

  • Baseline: sensitive estradiol (the sensitive assay matters for men), total and free testosterone, lipids
  • After any dose change, 1-2 weeks later: re-check sensitive estradiol
  • Periodically: lipids; consider bone density for long-term use
  • The goal is estradiol in a healthy range, not as low as possible

Realistic expectations

  • A little goes a long way; this is a drug you titrate carefully
  • The aim is balance, not zero estrogen
  • Symptoms of low estrogen often mimic the problems people are trying to fix

Common mistakes

  • Dosing reflexively without measuring estradiol
  • Using a standard (non-sensitive) estradiol assay in men
  • Chasing the lowest possible estrogen
  • Ignoring joint, mood, and libido signs of a crash

A tracking template

  • Pre: baseline sensitive estradiol, testosterone, lipids
  • On: daily symptom logs, re-test sensitive estradiol 1-2 weeks after any change
  • Long-term: monitor lipids and bone health

Safety notes

Over-suppression causes bone density loss, joint pain, low libido, poor mood, and adverse lipids. Use the sensitive estradiol assay and titrate against symptoms and labs, not by habit. Prescription-only; manage under a physician.
